JOB SUMMARY
Presentation Rehabilitation and Skilled Care Center, a contracted veterans nursing home providing skilled nursing and rehabilitation in Brighton, MA, is seeking a licensed practical nurse. The successful candidate will be responsible for providing direct patient care, collaborating with the interdisciplinary team, and ensuring that patient care plans are followed. As a licensed practical nurse, you will be required to maintain accurate and complete medical records, administer medications and treatments, and assist with daily living activities.
SALARY
$30 – $39/hr – commensurate with experience

JOB RESPONSIBILITIES
- Provide direct patient care and assist with daily living activities
- Administer medications and treatments as prescribed
- Educate patients and their families on care plans and treatments
- Collaborate with the interdisciplinary team to develop and implement patient care plans
- Maintain accurate and complete medical records
- Monitor and report patient progress and changes in condition
QUALIFICATIONS
- Current and Valid MA LPN license required
- BLS Certification
- Prior experience with geriatrics or long term care preferred
- Nursing Degree
